What Is GBT? Guided Biofilm Therapy for Gum Health: Understanding Its Role

Guided Biofilm Therapy, or GBT, is a modern dental hygiene system developed by EMS. It’s designed to make professional cleaning more comfortable and efficient by targeting dental biofilm using disclosing agents, and minimally invasive instruments.

We’re often asked: “Is GBT worth the investment?” It’s important to note that GBT is a commercial system exclusive to EMS. However, traditional hygiene techniques like scaling and the use of disclosing tablets, remain highly effective for maintaining gum health. GBT can enhance the patient experience and support care, but it’s not a replacement for consistent oral hygiene or comprehensive periodontal management.

What Is GBT?

GBT, or Guided Biofilm Therapy, is a modern approach to dental hygiene that focuses on the identification and removal of dental biofilm. Using disclosing agents, air polishing, and minimally invasive instruments, GBT aims to disrupt biofilm on tooth surfaces and within accessible periodontal pockets in a gentle, patient-friendly way. 

When used appropriately, GBT can offer several benefits:

  • A more comfortable experience for patients
  • Improved efficiency during hygiene appointments
  • Effective disruption of biofilm on teeth and shallow subgingival areas

Some clinical evidence shows that GBT protocols achieve comparable reductions in plaque and gingival bleeding to traditional scaling, with improved patient comfort. [1] For many patients, this can mean a more positive and less invasive hygiene visit. However, while GBT is a valuable tool, it’s important to understand what it can and cannot do.

Understanding The Limitations of Guided Biofilm Therapy

While GBT can improve comfort and efficiency, it does not replace the fundamentals of periodontal care. Mechanical cleaning alone cannot fully address advanced gum disease, which is a chronic inflammatory condition influenced by tissue-invasive bacteria and systemic health factors. GBT is another tool but patient education, daily hygiene, and overall health are equally important. We work on your nutrition and lifestyle factors to reduce your overall inflammation. 

It’s important for patients to understand that while GBT is innovative, other methods, like traditional scaling and disclosing tablets, remain highly effective and accessible.

Our London dental hygienist Brooke Shipp says Guided Biofilm Therapy is a useful and effective approach, particularly in terms of patient comfort, education, and biofilm visualisation. However, I don’t see it as a fundamentally new or superior standard compared with a thorough, well delivered hygiene appointment. 

In my practice, I use what’s commonly referred to as “airflow,” which is a gentle spray of bicarbonate powder, air, and water to remove soft plaque (biofilm) and surface staining, alongside ultrasonic instrumentation, hand scaling, and polishing where appropriate as part of comprehensive periodontal care. 

I use the ultrasonic scaler and hand scalers both above and below the gumline when indicated, as this remains important for effectively disrupting biofilm and removing calculus. 

Overall, I see GBT as a structured and well marketed protocol rather than a replacement for sound clinical judgement and thorough debridement. I’ve also found that using a microscope to show patients the bacteria in their own plaque can be far more motivating than using a disclosing solution alone, actually seeing what’s happening inside their mouth has been a real game changer for patient understanding and engagement.

Understanding Periodontal Disease

Periodontal disease is often misunderstood. It is not caused solely by plaque or biofilm sitting on the surface of the teeth. While surface biofilm plays a role, periodontal disease is driven by pathogenic bacteria that can become tissue-invasive, meaning they migrate beyond the tooth surface and into the gum tissue itself.

Once bacteria invade the tissue (leaky gums), mechanical disruption alone, even beneath the gumline, is no longer sufficient as a standalone treatment. This is where the misconception around GBT often arises.

A Holistic, Patient-Centred Approach

  • Oral Microbiome testing. In depth laboratory analysis of bacteria, protoza and viruses in your mouth.
  • Blood markers.  Looking at CRP (levels of inflammation in your body, HbA1c (average blood glucose control over past three months) and Vitamin D (master of Immune system).
